1.1.3 Accidents, Injuries, and Defects

Anything encountered that is out of the ordinary should be reported to the proper authority by the first available means of communications. Out of the ordinary can include, but is not limited to, accidents, injuries, defects in track, bridges or signals, defects in equipment or any other condition that affects the safe and efficient operation of the railroad.

Sometimes one of these problems requires a written report. If a report is required, it should be provided as soon as possible after the problem is reported while it is still fresh on your mind.
